A Whole-Person Approach to Active Treatment, Recovery and Life After Cancer

Finishing cancer treatment is an extraordinary milestone, but it is not necessarily the end of cancer care. For many people, it marks the beginning of an entirely different phase of health management. Cancer and its treatments can leave lasting effects on the cardiovascular system, muscles, bones, metabolism, cognition, sleep, emotional health, sexual health, nutrition, mobility and overall stamina. Survivors may leave active treatment cancer-free yet continue to experience fatigue, weakness, neuropathy, weight changes, lymphedema, anxiety, hormonal changes, cognitive difficulties and uncertainty about what they should be doing next.
The Cancer
Survivorship & Recovery Program is envisioned as a
comprehensive cancer wellness program built around two interconnected missions:
helping people remain as strong,
active and nutritionally supported as medically appropriate during cancer
treatment, and helping survivors rebuild and protect their health following
treatment.
The philosophy is simple: Cancer care should not end when treatment ends—and
wellness should not have to wait until treatment is over.
TWO
PATHWAYS. ONE CONTINUUM OF CARE.
PATHWAY
I — STAYING ACTIVE DURING CANCER TREATMENT
Maintaining
Strength, Movement and Quality of Life During Treatment
Historically, people undergoing cancer treatment
were often encouraged primarily to rest. Rest remains important, but cancer
rehabilitation increasingly recognizes that appropriately prescribed physical
activity can play an important role in maintaining function and quality of life
during treatment.
This pathway provides individualized, medically
appropriate exercise and supportive wellness strategies for people undergoing
chemotherapy, radiation, immunotherapy, hormonal therapy, surgery and other
cancer treatments.
This is not conventional fitness training. It is cancer-informed exercise and wellness support,
coordinated whenever appropriate with the patient's oncology and medical team. Programs
may include aerobic conditioning, resistance exercise, flexibility, balance,
mobility, breathing exercises, restorative movement and progressive strength
training. Exercise recommendations should be modified according to treatment
schedules, fatigue, blood counts, surgical restrictions, neuropathy,
cardiovascular considerations, bone health and other clinical factors.
The objective is not athletic performance. The
objective is preservation—preserving
strength, mobility, independence, confidence and quality of life while the
individual moves through treatment.
PATHWAY
II — POST-TREATMENT CANCER RECOVERY
Rebuilding
Health After Treatment
The second pathway begins when treatment ends—or
whenever the clinical team determines that the individual is ready to
transition toward longer-term recovery. Post-treatment cancer recovery is
organized around a question many survivors eventually ask: “What can I do now to become
healthy again?”
The answer should be more comprehensive than simply
returning for surveillance scans. Post-treatment survivorship requires a
personalized roadmap addressing the physical, nutritional, psychological and
lifestyle consequences of cancer treatment while helping survivors reduce
modifiable health risks and pursue the best quality of life possible.
The program therefore organizes survivorship around
several interconnected areas of health and recovery.
1.
EXERCISE ONCOLOGY & PHYSICAL RECONDITIONING
Every participant should begin with an appropriate
assessment of strength, mobility, balance, cardiovascular capacity and
functional limitations. A progressive exercise oncology program can then
address deconditioning, muscle loss, reduced endurance, treatment-related
fatigue and loss of confidence in physical activity.
Exercise programming may incorporate cardiovascular
conditioning, resistance training, flexibility, mobility, balance and
functional movement based upon the individual's condition and medical
circumstances.
The goal is to move the survivor progressively from
recovery to conditioning,
conditioning to strength, and strength to sustainable lifelong activity.
2.
CANCER-INFORMED NUTRITION
Nutrition after cancer should not be built around
fear, miracle foods or restrictive trends.
Registered dietitians or appropriately credentialed
nutrition professionals can help survivors develop sustainable eating patterns
supporting adequate protein intake, healthy body composition, metabolic and
cardiovascular health, bone health and overall nutritional adequacy.
Where appropriate, nutritional support can also
address treatment-related appetite changes, gastrointestinal difficulties,
swallowing problems, altered taste, unintended weight loss, weight gain and
other challenges that may continue following treatment.
The objective is not simply to create a diet. It is
to help survivors establish a
sustainable nutritional foundation for recovery and long-term health.
3.
RECURRENCE-RISK REDUCTION & PREVENTIVE HEALTH
No responsible survivorship program should promise
to prevent cancer recurrence. It can, however, help survivors address modifiable health and lifestyle factors associated
with better overall health and, for some cancers, improved outcomes.
This component emphasizes physical activity,
healthy weight management where appropriate, tobacco avoidance, alcohol-risk
education, nutrition, adherence to prescribed medications, recommended cancer
surveillance and management of cardiovascular and metabolic risk.
The message should be empowering rather than
frightening: We
cannot control everything about cancer, but we can help survivors take greater
control of the health they have today.
4.
CARDIOVASCULAR & METABOLIC HEALTH
Cancer survivorship is increasingly connected to
long-term cardiovascular and metabolic health.
Depending upon treatment history and individual
risk, survivors may benefit from monitoring blood pressure, lipids, glucose,
body composition, cardiovascular fitness and other clinically appropriate
indicators.
This becomes particularly important for people
exposed to potentially cardiotoxic therapies or treatments associated with
hormonal and metabolic changes.
Survivorship therefore becomes an opportunity not
only to monitor for cancer recurrence, but also to address the individual's whole-health risk profile.
5.
STRENGTH, BONE HEALTH & MOBILITY
Cancer treatment can contribute to muscle loss,
weakness, frailty and changes in bone health.
Resistance training, weight-bearing activity,
balance work, mobility exercises and appropriate nutritional support can
therefore become important components of long-term survivorship.
The objective is bigger than fitness. It is about
maintaining the physical reserve necessary for independence, mobility, resilience and healthy aging.
6.
LYMPHEDEMA, NEUROPATHY & PHYSICAL REHABILITATION
Some survivors continue living with consequences of
surgery, chemotherapy or radiation long after treatment ends. Access to
oncology rehabilitation may include physical therapy, occupational therapy,
lymphedema assessment and management, neuropathy support, mobility
rehabilitation, scar and tissue management and strategies for returning safely
to normal activities.
Rather than asking survivors simply to adapt to
these limitations, the program encourages appropriate evaluation and
rehabilitation wherever improvement may be possible.
7.
SLEEP, STRESS & EMOTIONAL RECOVERY
Cancer affects more than the tumor. Fear of
recurrence, anxiety, depression, sleep disruption, altered identity and the
psychological transition away from intensive medical care can profoundly affect
survivorship.
Behavioral health, stress management, mindfulness,
peer support and sleep-health strategies should therefore be integrated into
the survivorship model rather than treated as optional extras.
Emotional recovery should be recognized as an
important component of physical recovery.
8.
SEXUAL, HORMONAL & INTIMATE HEALTH
Cancer and cancer treatment can affect menopause,
testosterone, fertility, body image, pelvic health, sexual function and
intimate relationships. These subjects are frequently under-addressed despite
their importance to quality of life.
Survivorship care should provide an appropriate
pathway for discussing these issues openly and obtaining specialized clinical
support when needed. Quality of life includes the ability to reconnect with
one's body, relationships and sense of identity following cancer.
9.
COGNITIVE HEALTH — “CHEMO BRAIN” AND BEYOND
Problems with concentration, memory, processing
speed and mental fatigue can interfere with work, relationships and everyday
life. Survivors experiencing cognitive changes can be offered assessment,
education, compensatory strategies, cognitive rehabilitation where appropriate,
exercise and sleep interventions, and referral for further evaluation when
necessary.
The objective is to help survivors regain
confidence in their cognitive abilities and provide practical strategies for
managing persistent difficulties.
10.
RETURNING TO WORK, FAMILY & EVERYDAY LIFE
Perhaps the most important part of survivorship is
not clinical at all. It is helping someone become a person again—not simply a
patient. Returning to work. Traveling. Exercising. Rebuilding relationships.
Reestablishing routines. Regaining confidence. Developing new goals. Managing
fear without allowing fear to dominate everyday life.
Some survivors may also need vocational support,
workplace accommodations, financial navigation, caregiver and family support or
assistance adapting to a changed physical capacity.
This is where quality of life becomes a meaningful objective of cancer care.
AN
INDIVIDUAL SURVIVORSHIP HEALTH PLAN
Every participant should ultimately leave with an Individual Survivorship Health Plan—a
living roadmap developed around the individual's cancer history, treatment
exposures, present health, functional needs and personal goals.
The plan may incorporate:
- Exercise and physical activity recommendations
- Nutritional priorities
- Rehabilitation needs
- Cancer surveillance and recommended medical
follow-up
- Cardiovascular and metabolic considerations
- Bone-health considerations
- Lymphedema and neuropathy management
- Sleep and fatigue strategies
- Cognitive-health support
- Emotional and behavioral-health resources
- Sexual and hormonal health considerations
- Healthy weight and body-composition goals
where appropriate
- Tobacco and alcohol risk reduction
- Social, family and caregiver support
- Return-to-work and functional goals
- Personal quality-of-life objectives
Progress can then be reassessed at defined
intervals—for example, at baseline, 90 days, six months and one year. This
transforms survivorship from a vague instruction to “take care of yourself” into an
organized program of recovery and long-term health management.
A
MULTIDISCIPLINARY SURVIVORSHIP TEAM
Cancer recovery cannot realistically be addressed
by one professional discipline.
A comprehensive survivorship program can bring
together oncology, primary care, exercise oncology, physical therapy,
nutrition, behavioral health, rehabilitation, lymphedema specialists,
sexual-health professionals and other specialists according to the needs of the
individual.
The oncologist does not need to become the
patient's nutritionist, exercise physiologist or rehabilitation therapist. Instead,
the program creates a coordinated
pathway connecting survivors with the right expertise at the right stage of
recovery.
MEASURING
MORE THAN SURVIVAL
Cancer programs understandably measure survival,
disease progression, recurrence and treatment response.
A survivorship
program should measure something additional: How well is the survivor living?
Depending upon the program and clinical setting,
outcomes may include changes in strength, cardiovascular fitness, mobility,
fatigue, sleep, body composition, functional capacity, emotional well-being,
return to work, adherence to physical activity and overall quality of life.
These measurements can demonstrate that
survivorship is not merely a philosophy. It can become a structured and measurable component of cancer care.
THE
GREATER VISION: SURVIVORSHIP IS A PHASE OF CARE
The larger opportunity is to redefine what society
means by a cancer survivor. Survivorship should not merely mean that treatment
stopped. It should mean that another
phase of care has begun. Staying Active During Cancer Treatment
helps individuals preserve as much physical and emotional capacity as medically
appropriate while undergoing treatment.
Post-Treatment Cancer Recovery helps individuals rebuild that capacity afterward.
Together, these pathways create a continuum extending from diagnosis and active
treatment through rehabilitation, survivorship, prevention-focused health
management and healthy aging.
The program does not promise that exercise,
nutrition or lifestyle will prevent cancer from returning. Instead, it gives
survivors something equally important: evidence-informed
ways to participate actively in their own recovery, address modifiable risks,
rebuild physical reserve and improve the quality of the life they fought so
hard to preserve.
Cancer
treatment has traditionally centered on an essential question: “How do we treat this
cancer?”
A comprehensive survivorship program adds another: “How do we help this person live as well as possible during treatment—and how do we help them become healthy, strong and active afterward?”
That is the mission of the Cancer Survivorship & Recovery Program. The objective is not simply surviving cancer. It is helping people live well during treatment, recover after treatment, protect their future health and reclaim the fullest quality of life possible.
